Causes of hair loss in men

It is normal for a person to lose between 100 and 150 strands of hair per day, but in a normal cycle, these are constantly replaced by new follicles. However, with age, some men lose many more strands than are replaced in the growth and shedding cycle. This is the initial stage of balding, which progresses over the years. The most common cause of hair loss in men is androgenetic alopecia, a hereditary form of male-pattern baldness.

Other types of hair loss include:

  • Trichotillomania: a nervous disorder involving the compulsion to pull out one’s own hair.
  • Alopecia areata: an autoimmune disorder that causes round patches of hair loss and can lead to total baldness.
  • Alopecia totalis: a chronic condition of complete hair loss on the scalp, affecting a small percentage of patients with alopecia areata.
  • Hair loss caused by certain medications or conditions such as anemia or thyroid disease.
  • Trauma.
  • Chemotherapy.

What causes hair loss in men?

Hair loss can be due to various conditions, including hereditary male-pattern baldness, hormonal changes, aging, certain medical conditions, or periods of stress. It can also occur as a side effect of many types of medications, such as those for cholesterol, anticoagulants, blood pressure medications, psoriasis treatments, certain antacids, some steroids, and antifungal medications. Radiation therapy can also lead to hair loss.

Hair growth cycle

Hair typically grows about 1 cm per month, and each follicle is independent. This means that all hair follicles go through the same growth phases but at different rates. Hair grows from the follicle beneath the skin and is nourished through the blood vessels at the base of the follicle, which provide all the nutrients needed for growth. The hair growth cycle goes through 3 phases before falling out, years later.

Growth stages:

Anagen: This is the longest phase and typically lasts between a minimum of two years and a maximum of eight. Most follicles are in this phase (about 90%). The anagen phase is the growth phase, during which healthy hair is built.

Catagen: The transition phase during which hair follicles shrink. This phase lasts approximately two weeks, during which the hair stops growing and disconnects from the blood supply.

Telogen: This is the resting phase and lasts for several months. During this time, the follicle is not producing hair, and the hair on the scalp is shed. The thickened hair rises through the hair follicle until just below the entry of the sebaceous gland and eventually, this hair is shed. Depending on the hair and its location, it typically falls out within one to three months.

Exogen: The exogen phase is the final stage of the hair cycle and is essentially an extension of the hair’s resting phase. Eventually, the dead hair falls out, and new hair begins to grow simultaneously. Under normal conditions, between 50 and 100 hairs are lost daily during this phase.

In a person without alopecia issues, the distribution of the phases is as follows: 85% in the anagen phase, 1% in the catagen phase, and 14% in the telogen phase.

Why do so many men experience male pattern baldness?

It has been reported that up to 95% of men with thinning hair have male pattern baldness, which is hereditary. Experts suggest that a gene may affect the sensitivity of hair follicles to a hormone called DHT, which causes them to shrink. As they become smaller, the hair that grows back tends to be finer and thinner until it eventually ceases to be produced.

At what age can I start experiencing hair loss?

There is no specific age at which men start to lose their hair. Male pattern baldness (hereditary) can begin even during adolescence. However, initial thinning is usually noticed in the 30s or 40s. The earlier it starts, the greater the loss is likely to be. It is best to address this condition in its early stages.
